Via a referral form
– Once this form has been completed we will arrange
to meet you to do an individulised assessment with one
of our experienced staff members.
Once the assessment has been carried
out and you are in receipt of Direct Payments or Independent
Living Fund you will be invited to spend some time looking
around the centre before deciding what facilities you
would like to benefit from.
After this time a timetable will be
set up with yourself of how you would like to spend
your days when at the Amber Centre, this can always
be changed as and when you desire.
